Tetanus, diphtheria, pertussis (Tdap)one (1) time booster after 6/2005 required for all incoming students.If Tdap date is > 10 years from date of enrollment, a recent Td vaccine date must also be included.
MMR (measles, mumps, rubella)

two (2) doses (at least four weeks apart beginning at or after 12 months of age)

Laboratory proof of immunity is acceptable.
Hepatitis B seriesthree (3) doses at varying intervalsLaboratory proof of immunity is acceptable.
Varicellatwo (2) doses (at least four weeks apart beginning at or after 12 months of age)Laboratory proof of immunity is acceptable.
Meningococcal conjugate quadrivalent (A,C,Y,W-135) (Menactra, Menveo or Nimenrix only)one (1) doseFor all full-time students 21 years of age and younger. The vaccine must be administered on or after the 16th birthday. A signed waiver is also acceptable. 
Tuberculosis N/ACompletion of TB screening test form required; additional testing if necessary. Connell Graduate School of Nursing also requires annual PPD testing.
Influenzaone (1) doseThe seasonal influenza vaccine for the current flu season (July-June) must be received annually by December 31st.  New students entering between January 1st and March 31st must have received a dose of vaccine for the current flu season for entry.
