Meningococcal conjugate quadrivalent (A,C,Y,W-135) (Menactra, Menveo or Nimenrix only) | one (1) dose | For all full-time students 21 years of age and younger. The vaccine must be administered on or after the 16th birthday. A signed waiver is also acceptable. |
Tuberculosis | N/A | Completion of TB screening test form required; additional testing if necessary. Connell Graduate School of Nursing also requires annual PPD testing. |
Influenza | one (1) dose | The seasonal influenza vaccine for the current flu season (July-June) must be received annually by December 31st. New students entering between January 1st and March 31st must have received a dose of vaccine for the current flu season for entry. |
