Medical Forms
All mandatory health forms can be found below or on your Agora Health Services Portal. Massachusetts immunization requirements apply to all international students, full-time and part-time health science students, full-time undergraduate students, and graduate students.
Important: Please log on to your portal prior to coming to campus to ensure all immunizations are entered and mandatory forms have been completed, including the TB Questionnaire required of all students. Failure to do so may result in accounts holds and additional fees.
Allow 7–15 days for the retrieval, copying, and forwarding of your medical records. Copies of records will not be faxed but will be mailed or can be picked up at Health Services during normal operating hours.
This form should be filled out by students who have graduated from BC or no longer attend.
Active students can print their immunization records from the Student Health Services Portal.
