

The overall objective of the I-CARE4OLD project is to develop high quality decision support for better prognostication of health trajectories, including treatment impact, in older care recipients with complex chronic conditions using linked real world data.

Secondary analysis with use of existing data from a comprehensive assessment of patient characteristics and patient journeys; data is routinely collected for nursing home and home care clients. Machine learning strategies will be used to develop algorithms for risk prediction and identification of best treatment approaches.
Project impact - better informed decision making for clinical management of older care recipients with complex chronic conditions in home care and nursing homes through:
(1) high-quality, internationally validated, predictive algorithms on disease trajectories and treatment outcomes
(2) a multinationally tested e-platform for health professionals to receive predictive scenarios on course and treatment outcomes of newly assessed care recipients at point of care;
(3) dissemination among health professionals in nursing homes and home care
(1) highly reliable, valid and harmonized comprehensive assessments of functional capacities, diseases, and treatments, linked with
administrative repositories on mortality and care use
(2) develop and validate decision support algorithms using a variety of techniques including machine learning to improve prediction
(3) improve prediction of the outcomes of death, hospitalization admission, clinical disability, functional decline, cognitive decline and health-related quality of life
(4) examine the modifying impact of pharmacological and non-pharmacological treatments
(5) utilizes longitudinal data from 52 million older recipients of home care and nursing home care from eight countries.
